When you request a quote, we collect the coverage range, state, age, gender response, nicotine or tobacco response, name, phone number, email address, and contact consent you submit. Please do not send Social Security numbers, payment details, medical records, or other information this form does not request.
2. Website and call information
The site may use an essential session cookie to protect forms and the private staff inbox. If CallRail tracking is active, it processes call-related information such as the dialed number, caller number, time, duration, and associated website attribution. Calls are not recorded by this website’s form system. Any call-recording notice is provided in the telephone flow.
3. Optional Google measurement
With your permission, we use Google Analytics and Google Ads measurement to understand public page visits, where visitors arrive from, and call-button selections. Google may process cookie and device identifiers, IP and browser information, page details, and approved advertising click or campaign identifiers.
